Completed Player Contract - 2013 Physical Form - 1 copy of birth certificate
2 copies of final year report card on 8.5” x 11” paper

Turned in at walk-in registration or at designated paperwork turn-in TBD dates

MVPW has a League Raffle Ticket Fundraiser
Families will have the opportunity to sell a chance to win a pair of season tickets for the San Diego Chargers.
$50 per player which will be collected at the time of registration and are now included in the new registration fees.
Each raffle ticket (10 total) can be sold for $5.00. Monies collected are to be kept by each family for a potential $50 reimbursement per participant.

Refund Policy:All requests for refunds must be made, in writing, and received by May 1st, 2013 at 5pm. All refunds will have $100.00 deducted for Administration Cost. No refunds will be issued after May 1, 2013.
